This video presents a tractional retinal detachment repair involving fibrovascular proliferation to the anterior vitreous using a subretinal bleb. A 36-year-old woman presented with uncontrolled proliferative diabetic retinopathy and vision loss for more than 1 year. Examination revealed stalklike fibrovascular proliferation from the optic nerve to the anterior vitreous, as well as subretinal bands and a retinal fold. Despite removal of epiretinal and subretinal membranes, the retinal fold remained taut. Using a 41-gauge needle, balanced salt solution (BSS) was injected into the subretinal space to induce a retinal bleb. Further membrane peeling was undertaken, followed by an air–fluid exchange. The retina was noted to flatten nicely, with resolution of the retinal fold. A yellowish subretinal lesion was noted in the papillomacular junction.
