[Highlight at 12:04] All-Femtosecond Refractive Surgery
The femtosecond laser may have begun its ophthalmic odyssey as a high-tech microkeratome, but today it can do "all-in-one" refractive surgery, with nary an excimer laser or a corneal flap in sight. In 485 eyes, Indian surgeon Rupal S. Shah, MD, has corrected myopia by using a femtosecond laser to carve a precise, 6mm-wide lenticule in the stroma, then removing it through a 4-5mm curved incision (or by peeling it after lifting a LASIK-style flap). In eyes with moderate-severe keratoconus, collagen crosslinking combined with implantation of intracorneal ring segments (after injecting riboflavin into the tunnels) might improve vision more than either procedure does alone. Based on her studies in Turkey, Aylin Kiliç (Ertan), MD, will suggest keeping the interval between the implant and crosslinking procedures brief -- preferably a matter of minutes, instead of weeks or months.

[Highlight] Femto-Everything?
The femtosecond laser will be discussed for everything from its original use, cutting LASIK flaps, to highly specialized uses such as Descemet-stripping endothelial keratoplasty. But early clinical results with adding the femto- mystique to the procedure that is the bread and butter of most ophthalmic surgeons -- cataract surgery -- also are on the agenda. Reports on ongoing international trials, with followup as long as one year, will come from Zoltan Nagy, MD, Jonathan H. Talamo, MD, and Stephen G. Slade, MD.

A. John Kanellopoulos, MD, of Athens, also sees a synergistic effect from same-day crosslinking and refractive correction, but his preferred modality is topography-guided PRK. His comparison of 325 keratoconus patients followed for two to five years showed they had better vision and more stable corneas if PRK occurred on the same day as the crosslinking, compared to an interval of six months between procedures. His experience with the combined technique now extends to more than 600 cases.
