  • Correct NDC for Avastin from a Compounding Pharmacy


    We purchase Avastin from a compounding pharmacy. Should we use the national drug code (NDC) number on the repackaged syringe or the NDC number for the medication for billing?

    Answer:
    Report NDC 50242-0060-01 for Avastin. Correct billing is based on the medication NDC prior to repacking. The NDC from a compounding pharmacy is for tracking and not billing purposes. Using it on a claim will prompt denials.

    Additional denial issues often seen with compounded Avastin:

    • The unit of measurement (UOM) should be ML0.05 (document in item 24a of the CMS 1500 form, following the NDC).
    • The dosage and volume for Avastin is 1.25 mg/0.05mL and should be included in item 19.
